MSM Faculty, Staff, and Students must register by 11:59 PM on March 31, 2016 in order to be eligible for the gratis registration. All MSM Faculty, Staff, and Students must register using your unique msm.edu email address and provide proof of status. Acceptable proof includes valid student/Staff ID, letter of enrollment on school letterhead, OR registration/enrollment receipt from your school. MSM Rate includes one program at-a-glance and access to conference activities. 5 Student Rate includes one program at-a-glance and access to conference activities. Students are classified as Post-Doc Fellows, Undergraduate students, and Graduate students. Students must provide proof of status. Acceptable proof includes valid student ID, letter of enrollment on school letterhead, OR registration/enrollment receipt from your school. The document must show your name, the full name of your school, and an unexpired date (showing that you are currently a student). 6 Local organizations must provide proof of their 501(c)3 status to take advantage of the Community Exhibitors rate. When registering online for the Public Health Summit, you have the opportunity to pay by Credit Card or Institution Purchase Order/Check.

NOTE: If payment is not received by the respective deadline, then your balance due will be adjusted according to the timeline and respective rates as noted above.

ON-SITE PAYMENTS: Please note that neither Purchase Orders nor Personal Checks will be accepted on-site. Attendees must provide a valid Credit Card or a Business/Institution Check at time of Check-in to cover all Registration fees.

Cancellations

Notification of registration cancellations must be submitted in writing to the Public Health Summit Registrar. Registration cancellations occurring prior to Wednesday, March 23, 2016 will be assessed a 50% cancellation fee. No refunds will be granted for cancellations on or after March 23, 2016. Refunds will be distributed no later than 30 days after the close of the Conference. Neither the Conference nor its agents will be liable for canceled or delayed flights.

The request for a registration refund must include the tax identification number of the company or institution if registration was paid by a company or institution check.

Morehouse School of Medicine reserves the right to cancel activities prior to the scheduled date should uncontrollable circumstances make it necessary. Each registrant will be notified by the email address(es) provided during the registration process. In the case of activity cancellation, the liability of the Morehouse School of Medicine is limited to the registration fee.

Conference Management reserves the right to limit the number of participants in a program and is not responsible for any expenses incurred by an individual whose registration is not confirmed and for whom space is not available.

All attendees are required to indicate all special dietary or ADA accommodation needs when registering for the Summit.
